Dermal fillers in General
These are substances that are injected just below the surface of the skin to fill in lines, Wrinkles and scars.
Unlike face lift that stretches the skin, fillers add volume to the skin. It replaces age related volume loss of skin for a fuller natural youthful appearance.
It increases the fullness of the lips and lessen the downward slant of the corner of lips leading to lifts and creates happy look to your face.
Most common area treated are the smile line between the nose and mouth , fine lines above the mouth frown lines and acne scars as well as Lip augmentation. With certain fillers such as Sculptra and Radiesse we are able to reshape the face by adding volume to the cheeks and temples leading to a lifted look.
Type of dermal fillers
Volumenizing products:
Are those that add volume to the skin. The leading product is hyluranic acids such as Restylane, perlane and Juvederm.
Skin Stimulating products:
Are those stimulating collagen formation in the skin, such as sculptra.
Volume adding and stimulating products:
Are those adding volume and stimulates collagen formation such as Radiesse and Artefill.
How long the corrective effect of fillers last?
Temporary products are:
Hyluranic acids Family, Hyluranic acids attracts and holds water into skin. 6 to 12 month.
- Restylane and Perlane: most popular products filler.
- Juvederm and Juvederm ultra pluse.
- Captique: out dated old product– short lasting.
- Elavesses: newer product- hyluranic acid mixed with numbing solution- shorter lasting.
- Hylaform:used more for medical proposes- joint injection.
Calcium Hydroxyl-apatite class- last average one year
- Radiesse: lasts average one year or longer.
Poly-Lactic Acid class
- Sculptra: leasts average 2 tow years
Lipotransfer:
- Fat injection – Not a popular product due to unpredictable out come and risks.
Collagen last less than 3 month- out dated products
- Cosmoderm, Cosmoplast(Human collagen).
- Zyderm, Zyplast( Bovine-cow-collagen).

Permanent fillers:
- Silicone: not FDA approved in U.S.
Skin laxity, Aging and fillers
As we are aging, our skin gradually loses volume and its major constituents such as collagen, elastin and Hyluranic acid.
Collogen acts as the major support protein for our skin.
Elastin allows our skin to stay firm.
Hyluranic acid helps to trap water and add volume and shape to our skin.
What can be injected?
Collagen can be injected but it does not last very long. It can last only a few weeks to 2-3 months.
This is a filler used 10 years ago and earlier.
Elastin can not be injected but we can stimulate elastin and collagen formation in the skin with laser, deep acid peel, medical grade Microdermabrasion, topical products, Vitamins, etc.
Hyluranic acid is a natural substance that is available for injection as a dermal filler.

What area can be injected?
Most common area treated are the smile line which are creases from the nose to the corner of the mouth – Nasolabial lines- ( FDA approved)
Other areas commonly injected are: other smile lines around the mouth such as fine vertical lines of the upper lip (smoker lines), slant of the corner of lips (Merinet line),Fine line at the border of the upper and lower lip, Deep creases of the skin on the check, as well as frown lines, acne and Lip augmentation.

Does fillers injection hurts?
Dr. Shafa’s goal is to perform these procedures pain free. The discomfort associated with the injection can be controlled by multiple ways such as pre-medications with pills, nerve block, topical anesthesia, numbing cream, pre and post-icing, using cooling devise for numbing the area and more. Furthermore Arnica is offered to be used before and after injection which can significantly decreases the risk of bruising, one of the cause of discomfort after the treatment.
